Hypoparathyroidism - Definition, Causes, Symptoms and Treatment

Hypoparthyroidism is a deficiency of parathyroid hormone that causes abnormal metabolism of calcium and phosphorus. Underactive parathyroid glands produce too little parathyroid hormones, which in turn causes low levels of calcium in the blood stream. Parathyroid hormone (PTH) works in conjunction with vitamin D to regulate total body calcium. The body secretes PTH in response to hypocalcemia or hypomagnesemia; the hormone then stimulates osteoclasts to increase bone resorption. PTH also acts indirectly through the stimulation of adenyl cyclase to increase renal tubular calcium resorption and phosphate excretion. Hypoparathyroidism can result from congenital disorders, iatrogenic causes (eg, drugs, removal of the parathyroid glands during thyroid or parathyroid surgery, radiation), infiltration of the parathyroid glands (eg, metastatic carcinoma, Wilson disease, sarcoid), suppression of parathyroid function, HIV/AIDS, or idiopathic mechanisms. DiGeorge syndrome is a childhood disease in which hypoparathyroidism occurs due to a total absence of the parathyroid glands at birth.

Hypoparathyroidism is the only real result of parathyroid dysfunction and low PTH levels. This presents with tremor, tetany and, eventually, convulsions. HPTH can also be caused by surgery for Hyperparathyroidism (i.e. to remove overactive parathyroid(s) ). Approximately 3 or 4 percent of all patients with primary hyperparathyroidism will have an enlargement of all four parathyroid glands, a term called parathyroid hyperplasia. Hemochromatosis can lead to iron accumulation and consequent dysfunction of a number of endocrine organs, including the parathyroids. Absence or dysfunction of the parathyroid glands is one of the components of chromosome 22q11 microdeletion syndrome. Hypoparathyroidism UK is run for people with HPTH by people with HPTH. Hypoparathyroidism can result in hypocalcemia, and it commonly is diagnosed during the workup for hypocalcemia. Pseudohypoparathyroidism describes hereditary conditions that cause end organs to be resistant to PTH.

So there you are, rushing to catch a plane. You've got an important presentation on your laptop, along with sensitive company and personal information. And then, suddenly, it's gone.

Either you've lost it, or it's been stolen.

Airports and hotels are favourite places for thieves. Laptops disappear from both and that's bad enough. But often, irreplaceable data has gone with them. According to Safeware, an insurance agency specialising in insuring high-tech equipment, in the US alone 591,000 laptops vanished in 2001.

None of this has to happen to you. But you have to be extra careful today. With the increased security at airports, losing track of a laptop is easier than ever. Here are six guidelines to help you avoid seeing your notebook PC disappear - or, if it does, help you find it or get a quick replacement.

Tips for Getting Through Security Checks

Oddly, some airports have collected stacks of laptops. They're left behind by harried passengers in security areas. Most are reclaimed eventually. But some people never come back for their machines.

Obviously, you've got to allow yourself plenty of time when you fly. When I travel, I carry my laptop in a backpack. There's lots of room in there, and it's inconspicuous. I see fashionable businesspeople carrying laptops in expensive cases. They're asking for trouble. Those cases say, "Laptop! Laptop! Steal me!"

Security people most likely will want to check the laptop. If possible, handle it yourself, rather than having them do it.

Don't be afraid to speak up. You don't need your machine damaged.

Be sure your battery is charged. The security people may well want to turn on the machine. Most important, don't let it out of your sight. This becomes especially difficult if you are selected for a random head-to-toe check.

Once on board your flight, put the computer - safely stowed in your bag - under the seat in front of you. Try to avoid putting it in the overhead bin. Others will have more access to it there.

Have Sensitive Data? Encrypt It

If you have sensitive information on your computer, consider encrypting it. If you have Windows XP, you already have the tools needed. While the thief has your machine, these extra steps will make it much more difficult to access the laptop's data.

Encryption will protect your business secrets, but it won't retrieve your data. To do that, you've got to back up your information to another computer. If you run your own server, you can easily upload files to it.

You don't have to back up everything on the laptop. Just copy the files you will need to the server. If your laptop should be stolen on your trip, you may be able to borrow a computer at your destination and download your important files. Finding time for backups is a pain, I know. But they can save you.

Make Your MP3 Player do Double-Duty

If you don't have a server, or don't want to back up to it, take a look at MP3 players. Some today have huge hard drives. They are easily hooked to a laptop through the USB or FireWire ports. The data can then be copied to the MP3 player. These things can copy any data, not just MP3s.

Trace a Stolen Laptop's Location

If your laptop does get stolen, wouldn't it be great if the system could do the high-tech equivalent of phoning home? There are programs that will report the location of a stolen laptop. They work when the laptop connects to the internet.

Security experts also say that engraving your name on the computer is helpful. That will make it more difficult to sell on so thieves may look elsewhere.

Set a System Password

Every road warrior should protect their laptop with a start-up password. This way, a system password prompt will appear each time you start your business computer before Windows even begins to load. It will prohibit any access to the computer at all.

To create a system password, you'll need to access your computer's setup utility (also known as BIOS settings, which stands for "Basic Input/Output System"). Typically you do this by pressing Delete, Esc or F1 as soon as you turn on the computer. Check the manual though, as some computers are different.

Consider Laptop PC Insurance

If your laptop is expensive (and aren't they all?) you might want to insure it. Some homeowners, renters and business insurance policies don't cover computers, and if they do, there may be many conditions attached.

Today's laptops are ripe targets for thieves. If you follow these basic guidelines along with some good common sense, you won't have to give yours up.

E2 Visa FAQ and Answers for British Nationals

Q.Do I actually need an E visa to reside in the United States if I own a US business?

A.Unless you are a U.S. Citizen or a U.S. Green Card holder, you must be in possession of a valid have a visa in order to enter the United States in Treaty Trader or Treaty Investor status. Further, all successful E visa applicants and their dependents are expected to present valid passports in order to be issued the visa, regardless of nationality.

Q.I received a change of status in the United States from U.S. Citizenship and Immigration Services (USCIS). Is that all that I need to present in order to be issued an E visa at the U.S. Embassy or U.S. Consulate?

A.No. The change of status simply allows you to remain in the United States until the expiration of the status granted. If you have been granted a change of status by USCIS and leave the U.S., you must have an E visa in your passport in order to return to the U.S. in that status. To obtain a visa you must lodge a complete application with the appropriate U.S. Embassy or U.S. Consulate. Adjudication of your case can vary from two weeks to six months, depending on which U.S. Embassy or U.S. Consulate is deciding your case.

Q.How much money do I need to invest?

A.There is no minimum amount for an investment. E-2 visa regulations state that the investment must be sufficient to ensure success of the business. As different types of businesses require different amounts of capital, the amount you will need to invest depends on your U.S. enterprise.

Q.Do I really have to invest the money before I apply for the visa? Can't the United States government issue me the visa first?

A.E-2 visa regulations state that the funds must be "irrevocably committed" to the investment before the visa may be issued. Therefore, you must document that your investment meets this criteria at the time of initial application; this is usually accomplished by showing that the investment has already been made. Funds can be considered to be irrevocably committed, however, if they are held in an escrow account solely contingent on the issuance of an E visa.

Q.Must the business be trading at the time I lodge my E-2 visa application?

A.Yes. The relevant E-2 visa regulations state that the enterprise must be "real and active". Most U.S. Embassies and consular posts interpret this to mean that the business must be actively trading at the time your E-2 visa application is lodged.

Q. How can I possibly start a business if I don't have the visa?

A.You may enter the United States in B-1 (temporary business) visa status in order to set up (not run) your business. You may not be paid in the U.S. while in B-1 status. If your enterprise requires someone to manage or run daily operations, you may hire individuals who are already properly documented to work in the U.S. prior to receiving your visa. Once you have the initial commitments completed, you should apply immediately for the E visa.

Q.How long do I have to wait before I can apply for a "green card" or U.S. citizenship?

A.An E visa is a non-immigrant visa and does not lead to either a "green card" or U.S. Citizenship. You may remain in the U.S. only as long as your business conforms to E visa regulations, assuming you maintain proper visa and immigration status.

Q.Do I need an immigration attorney?

A.There is no legal requirement that you hire an attorney to lodge your E visa application. While many E Visa applicants choose to retain the services of an attorney to aid in the preparation of their case, others do not. It is to your advantage, however, to engage the services of a competent immigration attorney or law firm that is fully familiar with the specific procedures observed by the U.S. Embassy or U.S. Consulate in your home country. Failure to observe these procedures will certainly result in significant time delays, may seriously prejudice the outcome of your case, and may result in irreversible financial consequences.

Q.What licenses and permits do I need to open and run a business in the United States?

A.Licensing and permit requirements vary by state and county and with the type of business you wish to operate. Q.I already own an E company and want to employ someone who is not in the U.S. Can employees of an E company qualify for an E visa to work in my U.S. enterprise?

A.To qualify for an E visa as the employee of an E company, the applicant must have the same citizenship as the owners of the E company. Additionally, the job to be performed must be executive or supervisory in nature, or the employee must possess skills which are essential to the operation of the U.S. enterprise.

Q.Can my spouse and children work in the U.S.?

A.Effective January 16, 20002, dependent spouses of E visa holders are eligible to apply for work authorization from USCIS. Children of E visa holders are not permitted to work in the United States unless they independently qualify for employment authorization, such as an E, H, or L visa.

Q.My spouse (or child) uses a different surname than I do. Is that a problem?

A.A dependent whose surname differs from the surname of the E visa holder should have on-hand proof of the relationship (a marriage certificate for spouses or birth certificates for children, for example).

Q.Can my fianc(e), common law or same-sex partner accompany me?

A.Under U.S. immigration law, a legal marriage must exist before one is considered to be a spouse. Therefore, fianc(e)s, common law or same-sex partners do not qualify for derivative E visa status. Other avenues may be available to assist those applicants in such a situation. Please contact our firm for more details.

Q.My spouse and/or children are citizens of a country other than my own. Can they still accompany me?

A.The spouse and children (defined as unmarried and under 21 years of age) do not need to have the same citizenship as the principal applicant. However, dependents of E visa holders are required to have visas in order to accompany the principal applicant to the U.S.

Q.Will I have to appear before the U.S. Embassy or U.S. Consulate in person?

A.For all categories of visas, including Treaty Visas, each applicant age 14 or older must appear for a personal interview before a Consular Officer. In all cases, each applicant (including those under 14 years of age), must be physically present in the country of application at the time of issuance. Those applicants who are found to be ineligible for a U.S. visa for criminal convictions, immigration violations, drug charges, or other similar reasons may have to appear to determine grounds of ineligibility and applicability of a waiver for any such ineligibility. In such cases the applicant must be prepared for a wait of up to 180 days weeks while eligibility is confirmed and/or a waiver requested.

Q.How long does the processing normally take?

A.Processing times vary greatly between the various U.S. Embassies and U.S. Consulates. For example, the U.S. Embassy in Mexico City can process an E-2 visa case in one week, while the U.S. Embassy in London can take up to 6 months to process a case. Generally speaking, if your initial submission is not complete, then your case will not be officially "received" or it may be kicked back to allow you to provide the missing information. Upon resubmission of the case, it will generally be placed at the back of the processing queue. Please note that frequently an applicant's submission will require clarification or additional information before the adjudicating officer can make a determination of eligibility. If this is the case, you (or your attorney of record) will be notified in writing.

Q.Will it speed up the processing if I send my passport to the U.S. government at the time that I lodge my application?

A.No. You should not send passports until it is requested by the U.S. Embassy or U.S. Consulate. Submitting passports early can delay the processing of your case.

Q.I paid $100.00 USD when I applied. Why is there an additional fee for issuance?

A.The $100.00 USD fee you paid at the time of application is called the Machine-Readable Visa (MRV) processing fee. This non-refundable fee is charged to all applicants for non-immigrant visas regardless of whether the application is approved. Once your case has been approved, there may be an additional fee for issuance of the visa. This fee is called a "reciprocity fee" and is determined by the fees that your country of citizenship charges U.S. citizens for similar visas. The E visa reciprocity fee for a Canadian citizen, for example, is currently $40.00 USD.

The relevant treaty either may or may not allow one to lodge an application in ones home country.     Also as a bowler it is very important to stay in great shape, as youll enjoy the game more and perform better in the latter stages if you have good stamina. So incorporating some light weight sessions in to your training is important.

    Common Drugs and Their Uses

    The five drugs that are discussed in this article are Ibuprofen which is a non-steroidal anti-inflammatory drug (NSAID) that is commonly used for relief of Arthritis, Warfarin which is an anticoagulant drug commonly prescribed by doctors to treat venous thrombosis, pulmonary embolism, to treat or prevent dangerous blood clotting in people with arterial fibrillation and to prevent strokes, a benzodiazepine drug called Diazepam which is commonly prescribed by doctors for relieve nervousness, anxiety, and anxiety disorders, a sulfonylureas drug called Glipizide which is used to help control blood sugar levels and used to treat type 2 diabetes and a thyroid medication called Armour Thyroid which is prescribed for the treatment of hypothyroidism.

    Arthritis is a common illness in todays society; doctors prescribe drugs called non-steroidal anti-inflammatory drugs (NSAIDs) for this condition. A common NSAID is Ibuprofen which reduces inflammation, pain and high temperature and is used widely to relieve the symptoms of arthritis. Ibuprofen is available in prescription and nonprescription. As well as rheumatoid arthritis, Ibuprofen is used to treat mild to moderate pain, primary dysmenorrhoea, osteoarthritis, menstrual cramps, bursitis, gout, migraine headaches and fever.

    NSAIDs such as Ibuprofen can cause gastrointestinal (GI) irritation, bleeding, iron loss and kidney dysfunction and increased blood potassium levels, especially in older people. Ibuprofen may cause drowsiness, dizziness, or blurred vision especially when taken with alcohol.

    Ibuprofen has adverse results when taken with lithium, sodium and white willow.

    Lithium a mineral which is commonly prescribe to treat manic depression, many NSAIDs stop the excretion of lithium from the body which results in higher blood lithium levels which in tern can create unwanted side effects and interferes with efficacy. NSAIDs should be taken with caution for those people who are taking lithium whether in supplement form or for mood disorders. It is wise to reduce salt intake when taking Ibuprofen because Ibuprofen may cause sodium and water retention.

    People who are taking the herb white willow should be aware that links have been made that white willow reduces the blood level of NSAIDs.

    Warfarin is an anticoagulant drug commonly used to treat venous thrombosis, pulmonary embolism, treat or prevent dangerous blood clotting in people with arterial fibrillation and to prevent stroke.

    Coenzyme Q10, Green tea, Iron, Magnesium, St. Johns wort, Vitamin C and Zinc should be avoid when taking Warfarin as they can reduced drug absorption/ bioavailability. Also American ginseng, Asian ginseng, Cranberry, Dan shen, Devils claw, Dong quai, Fenugreek, Garlic, Ginger, Ginkgo biloba, Horse chestnut, Lycium barbarum, Papain, Quilinggao, Quinine, Red clover, Reishi, Sweet clover, Sweet woodruff, Vitamin D and Vitamin K should be avoided as they can cause dangerous or undesirable results.

    The long term of heavy drinking of alcohol can affect the efficiency of warfarin and also certain foods such as broccoli, Brussels sprouts, kale, parsley, spinach, which contain high amounts of vitamin K, can interfere with the effectiveness and safety of warfarin.

    Known side effects of warfarin are difficulty breathing, closing of the throat, swelling of the lips, tongue, face, hives, black, bloody, or tarry stools, blood in urine or vomit, coughing up blood, bleeding gums or sores in the mouth, decreased urine production, yellowing of the skin or eyes, severe dizziness or headache, easy bleeding or bruising under the skin, purple discoloration of the toes or fingers, sudden leg or foot pain, unusually heavy menstrual periods, excessive gas or bloating, diarrhoea, nausea, or vomiting, hair loss and decreased appetite or weight.

    Diazepam is commonly used to relieve nervousness, anxiety, and anxiety disorders. It is in a class of drugs called benzodiazepines which effect chemicals in the brain. People can become physically and psychologically dependent on diazepam, it is habit forming and withdrawal effects can occur when stopped. Diazepam can cause dizziness and drowsiness, alcohol can make these symptoms worst.

    Diazepam can harm unborn babies and can be passed into breast milk.     Adverse reaction can take place when Glipizide is taken together with fenugreek and Ginkgo biloba. Fenugreek has been proved in studies to reduce blood glucose levels in people with type 2 diabetes and ginkgo biloba worsened glucose tolerance in people who took oral anti-diabetes medication.

    Thyroid medication are used for the treatment of hypothyroidism and are either synthetic or animal-derived hormones, Armour Thyroid is a common example of thyroid medication. Calcium and Soya may reduce drug absorption and bioavailability within the body, bugleweed and lemon balm cause adverse reaction when taken with thyroid medication.

    Thyroid medication has been known to increase loss of calcium from the body through urine and calcium has been known to reduce the effectiveness of certain thyroid medication. Soya being taken at the same time as thyroid medication has been known to reduce it abilities.

    Bugleweed and lemon balm should not be used with thyroid medication as it is known that they interfere with the action of thyroid hormones.

    How To Sew Lined Shoe Bags For Golf, Tennis, Bowling, Gym, Dance or any Other Drawstring Bag

    Sewing shoe bags for the guys or gals in your life is a great gift idea. No matter what sport or hobby they enjoy or where they travel. . .far away or to the gym, shoe bags or drawstring bags come in handy.

    The how to sew sewing instructions are for a lined drawstring shoe bag.

    You are the designer. You can decide the purpose, fabric, design and size of your drawstring bag. Here are some suggestions.

    Fabrics

    Use whatever fabric suits your fancy. About 1/2 yard of fabric makes one shoe bag.

    Sturdier fabrics like denim, canvas, slipcover fabric, fake suede, corduroy, etc. work well for sturdier shoes.

    Fine fabrics like velour, velvet, cottons, flannels, etc. are good for dressier shoes.

    Think about satin or silk for lingerie bags or jewelry bags. Or use mesh or terry for the beach or laundry bags.

    Lining is optional. But, if you line the bags, choose a compatible lining fabric that won't tear easily especially for the sturdier shoes. Sometimes the same fabric is the best lining. The instructions here are for a lined drawstring bag.

    You may want to use washable fabrics so you can launder the bags later.

    Preshrink all fabrics before cutting.

    Drawstrings

    Use a pair of 36 inch long decorative shoe laces or leather laces for each bag or heavy cord or sew your own fabric or fake suede drawstrings. Ribbon also works.

    Sizes

    Sew a test bag to see if the dimensions are right for you.

    Cut one bag with the following suggested dimensions. Place the shoe inside and check to see if it fits well. Add or subtract from the length or width to adjust for whatever shoe or item that you have.

    Suggested Sizes

    For 2 golf shoe bags, cut 4 pieces of fabric 20 inches long and 11 inches wide. Cut 4 pieces of lining 19 inches long and 10-1/2 inches wide.

    Note: Cut an extra drawstring bag for a golf ball bag.

    Two bowling shoes will fit into one shoe bag. Cut 2 pieces of fabric 20 inches long and 11 inches wide. Cut 2 lining 19 inches long and 10-1/2 inches wide.

    One bag may also hold 2 tennis shoes or 2 ladies dress shoes.

    Gym shoes vary in size. For extra large or small shoes, measure around the shoe and adjust the size of the fabric plus seam allowances to accommodate the size of the shoe.

    How To Sew Sewing Technique

    Decorate the Shoe Bag

    Make the drawstring bag unique with a special design or emblem. Think about monogramming the bags or adding a sport motif applique or embroidery design prior to stitching. Place the design about 3 inches from the bottom in the center of one of the 20 x 11 pieces.

    After you trim the bag, place 2 bag pieces right sides together.

    Measure and make a mark 5 inches down from the top of the bag on both sides.

    Stitch 1/2 inch seam around bag starting and stopping at this 5 inch mark. Do not back stitch. For the first and last inch, use short stitches.

    Repeat stitching as above for the lining.

    Press all seams open on both bag and the lining.

    Press under 1/2 inch on the unstitched 5 inch bag side edges. Fuse or stitch to hold.

    Do the same for the lining.

    Turn the bag right side out.

    Turn the lining right side out.

    Machine stitch 1/2" at the bottom of the lining as if you were making a French seam. This creates a strong edge.

    Turn the lining wrong side out.

    Slip the lining down into the bag so that the top edge of the lining is 1/2 inch below the the top edge of the bag.

    Fold the top edge of the front of the bag down 1/2 inch over the lining.

    Press this fold to make a crease.

    Now, bring the top folded pressed edge of the bag down to meet the side edges at the bottom of the opening.

    Pin to hold and stitch close to this fold edge.

    To make the casing, stitch 1" above the previous stitching.

    Repeat for the back side of the bag.

    Insert Drawstrings

    Use a safety pin or bodkin to help slide the drawstrings through the casings.

    Slide one drawstring through the casing of the bag back and front having ends on the same side of the bag.

    Insert the other drawstring starting at the other side of the bag, sliding through the casing of the bag back and front having ends on that side of the bag.

    Tie the ends of the drawstrings together.

    That completes the lined drawstring shoe bag.

    Note: If the shoe bag is wide enough for two shoes, you may want to stitch up the center of the bag from the bottom up to (but do not include) the casing through all the layers to create separate compartments for each shoe to help keep them from scuffing.

    Pin before stitching and test to see if there is enough room for the shoes. If not, you may want to increase the width of your shoe bag.

    Sew bags any size for anything. Everybody needs a bag for something. Kids love treasure bags. Make big bags and little bags. Give drawstring bags to family and friends for Fathers' Day, birthday gifts, Christmas gifts, etc. Enjoy sewing bags!
